Housing Case Manager (HCM) at Steuben County DSS – Pro Action is accepting applications for the full-time position of Housing Case Manager to work at the Steuben County Department of Social Services.
This position requires the minimum of an associate degree in a field related to the provision of Social Services / Human Services, and 3 years of relevant experience, including providing case management.
The HCM must be self-directed, have strong organizational skills and the ability to manage multiple priorities and deadlines. Must be able to work independently while remaining an effective member of a collaborative team. The HCM must have solid written and verbal communication skills and excellent customer service skills. A valid driver’s license meeting agency standards is required for this position.
The Housing Case Manager will work collaboratively with Adult Protective Services (APS) and the BILT (Building Independence for the Long Term) Teams. A typical day for the HCM will be spent providing case management services to individuals who are receiving Emergency Solutions Grant (ESG) or Emergency Rental Assistance Program (ERAP) funds. You will conduct assessments to determine the goals of each client, their barriers to achieving their goals, and what additional resources may be helpful.
As you work with each client to develop and implement an Individualized Service Plan (ISP), you will include strategies for developing and achieving goals and addressing housing needs, vocational training, legal issues, and other factors that may impact the individual’s progress toward their goals. You will make sure clients are complying with the requirements of the grant funding they are receiving. You will maintain scheduled contact with all clients, as appropriate for their needs.
